I still cannot work out how I possibly took 3 damage here:
If you go frame by frame (using “,” and “.”), there is a single frame where you can see three coins.
Only one coin was reflected, so I don’t know how three appeared. (Maybe something weird happened because so many projectiles hit each other simultaneously? IDK)

Update from Discord chat! The bug has been discovered (apparently mirror can duplicate certain projectiles under certain circumstances) and has been fixed internally
